1. Not sure if possible to reproduce, but it was Swarmageddon
2. 3 cave complexity and cave length
3. Hazard 3
4. Gunk Seeds as optional objective
5. Crystalline Caverns

Explanation

Encountered an issue where the "Lift Pod" carrying the rocket thrusters for the new Heavy Extraction mission spawned in over 100m away, in the center of the original drop pod area (almost as if it used the drop pod coordinates to drop, not localized to the Resinite.
