Steps to Reproduce

1. Change focus during the wipe/loading screens
2. Play in windowed fullscreen mode with medium graphics settings
3. Run the game on a PC with an occasional frame hesitation, possibly related to the SSD or CPU
4. Use three monitors, with the third being a USB-HDMI splitter that might not draw enough power at times

Explanation

Not sure if it worked but I tried adding the screenshot I took of an odd visual bug after a mission failure. The top 1/3 of the display seems to be showing in the bottom-left corner of the screen for some reason.
