Adjust Grapple Damage to Scale with Distance for Balanced Combat

using grapple on an enemy within quick melee range should not deal lethal damage. this renders quick melee useless as it only deals 50 damage. It also renders the hammer useless since you dont need to swap weapons to one shot someone within melee distance. to fix this, have grapple damage scale with distance.
