Explanation
I died for no reason, I just walked and died, I have a video recording.

Dead barnacles have collision with the player as they fall, meaning it can launch you very quickly if you happen to be too close to them (similar to other objects like the barrels in the space rig. The high speed made "fall" damage lethal. Same issue as in #18
