Explanation

Lithophage meteorite swarm failed to spawn.

Was in Ossuary Depths, current patch, default launcher, running chapter 3: Plaguefall.

On-Site Refining mission, Lithophage infection in area, 1 spire, I had already neutralized. Mid-wave of swarmers, Kursite Grinder nearby but not turned on, 2 of 3 pumpjacks connected.
Heard meteorites break atmosphere, MC alerted me that meteorites were inbound. Meteorites never spawned.
Map was very small, only one room, and small even for that. Finished rest of mission without incident.
