Steps to Reproduce

1. I can't tell how to replicate it, my assumption is that it is related to spawning on top of the goo.

Explanation

The core stone spawned without it's "obelisk" above the goo, it stayed dormant, had no collision nor could have been hit. Jumping in it's place resulted in getting pushed into the cave level below.
