Steps to Reproduce

1. Complete hazard 5 dive in crystalline caverns
2. Check achievemnts on Steam
3. Achievement not unlocked

Explanation

On DRG Survivor achievements seem to be bugged for Hazard 5 completion.

I have cleared all stages but the achievements are not unlocked.
