Complete the generator inside the Prison Tower of Fallen Refuge
Escape the trial

Explanation

"Don't Open, Dead Inside" achievement is bugged.
After completing the generator inside the Prison Tower of Fallen Refuge and escaping the trial the achievement is not awarded.
I did it TWO times and didn't get the achievement. Frustrated af.
