Explanation

Weapons and shield disappear while holding in hand. My last two games (one last night and one today) I've had four pieces of equipment disappear out of my hand, a shield, torch and two weapons three in one game. Each time the sound effect that triggers when holding item to your chest and releasing grip to store in the backpack has played but the item wasn't moved into the backpack and I never released my grip button and I dont believe my hand was that close to my chest but I have had a few times where I hand grabbed my backpack off of my character when I haven't had my hand close to it so the area to interact with things might be bigger then I'd assume. I've checked the ground each time it's happened and haven't seen anything except when I lost my shield I didnt see it then I stepped back and noticed I could see it on the ground, I stepped forward and reached to pick it up and it disappeared. I can't say how to reproduce this but can say when I lost my torch and one weapon I was at the top of a ladder looking down kind of moving around it to get a view of the floor below and my torch disappeared a few second later I went to walk off and fall down the h*** the ladder was in and got hung up on the top of the ladder for a second and at the same time my weapon disappeared.
