Explanation
In the conquest map (Siberia?), near point Delta and Charlie, our tank fell through the map and was destroyed, killing the gunner and the driver.

So from what we can tell, the bug happens when two tanks shoot each other. The tank that dies first dies, but if the dead tank hits the other tank with their shell and the shell hits, the tank that should be alive gets shoved under the map.
