Steps to Reproduce

1. This happens at totally random times.
2. I'll attempt to note down the server name and time this happens next.

Explanation

My 3rd instance in 1 week were my bullet don't damage the enemy at point blank range. I'll attempt to forward the server name and time next time this happens, though tedious. And the glitch where shots are firing from your gun even though I'm not pulling the trigger is even worse now.
