Explanation
The Molaktula boss was able to softlock by not spawning the corestone fragments. During the fight, it successfully spawned the fragments twice though an enourmous lag spike (multi-second pause) would occur. Once the 3rd stutter occured, no corestone fragments spawned so the boss was perminantly invincible and the mission had to be aborted.
Side note, a corespawn rift (the spawn location thing) spawned inside the elevator where it lands on this same floor.
The Trident D.E.S has multiple audio persistance issues where the firing audio is persistant for observers (not using the gun) and this audio can stack. Also the visual projects are likely to persist stationary in the air for observers.
