Steps to Reproduce

1. I acquired CQC
2. died
3. was revived
4. attempted to attack with melee

Explanation

CQC disables permanently if you ever get downed. It also does not return by advancing floors, being revived, or filling back up on ammo to reset the bonus to 0%
