Explanation
Teammate got downed at the edge of the boss rift. He was out of reach of being revived because the rift kept pushing me away. He wasn't in the rift, but was beyond the boundary where you can walk without getting pushed back. He was also reviving another teammate when he was downed. Not sure if that matters to repeating the bug.
Game should knock his downed body away a bit or not pushback as aggressively.
