Steps to Reproduce

1. Enter a Heavy Weapon Crate modified level.
2. Collect and activate the toolbox and open the menu.
3. Do not collect any of the weapon upgrades.
4. Close the menu.
5. Collect the Heavy Weapon Crate.
6. Replace your weapon with the Heavy Weapon.
7. Return to the Toolbox to claim the upgrade for the Heavy Weapon.
8. The picture should display the previously held weapon instead of the Heavy Weapon.

Explanation

On a Heavy Weapon Crate modified level of a facility, if you collect and open the toolbox (without claiming the reward) then go and claim the Heavy Weapon to use on the toolbox, the toolbox will display the image of the weapon you had before the toolbox instead of that of the Heavy Weapon.
As far as I can tell, this is only a visual glitch. Selecting an upgrade for the replaced weapon applies the upgrade to the Heavy Weapon. (image unrelated, just happened to submit the bug after the run)
