Steps to Reproduce

1. using platform gun and a h*** next to the vault, wedged myself against the vault
2. phased out of the world and was teleported into the vault
3. was stranded in the vault and had to abort mission

Explanation

was using the platform gun to try and get out of a h*** next to a vault and ended up phasing into the vault and getting stuck inside
