Actual Result:	Change of Plan perk has an issue with Onryo VHS tapes. Using the perk deletes the tape item and restores toolbox charges instead of replacing with a med kit of equal rarity. After game screen shows a common med kid was received when it wasn't.
Expected Result:	Change of Plan perk should swap toolbox with med kit of same rarity without deleting the tape item. End game screen should accurately reflect what happened in-game.

Additional Notes & Comments:	I chose "once" for reproducibility since it happened to me once. I have not intentionally tried to reproduce it.

Steps to Reproduce

1. Have a depleted toolbox AND a tape from The Onryo
2. Enter a locker
3. Activate perk "Change of Plan"
4. Tape should vanish and toolbox charges should be restored

Explanation

The perk "Change of Plan" deleted Onryo VHS tape item and refilled toolbox charges instead of swapping for a med kit. I had a depleted toolbox and was also holding a VHS tape. I entered a locker, then pressed the action button to activate the perk... and then the tape vanished, and the charges on my toolbox were fully restored. The toolbox was usable on the final gen. I used the charges up again, then at the exit gate, I dropped the toolbox. At the results screen, the game stated I lost my toolbox but gained a common quality med kit, even though I never received one (and the rarity did not match the toolbox). My character was holding the med kit as well on this screen, even though I left the game empty-handed.
