I tried reproducing in the same map but had no luck only the one instance shown in the video

Steps to Reproduce

  1. Open a Nemesis Box
  2. Immediately grab the vaccine

Explanation

Opening a nemesis vaccine box and grabbing the vaccine CAN cause a server disconnect. It might be specific to the box and map but I wasnt able to find out.
