Steps to Reproduce

1. Get hooked on a hook that has an obstacle blocking one side of the hook (in end game)
2. Killer positions themselves right next to the hook to body block the survivors from unhooking
3. Fellow survivors will try to unhook
4. Survivors do not get the unhook prompt because the killer is body blocking
5. The killer is not swinging so they do not lose collision
6. The survivor dies on hook because no one is able to unhook them

Explanation

Due to the positioning of the hook and surrounding obstacles, the killer is able to body block the survivor from unhooking. The survivors do not get the unhook prompt. In end game especially, since there is no anti-camp feature, this is a guaranteed death with no counter.
