Additional Notes & Comments:	This issue seems to only affect addons; perks are working normally. In the screenshot example I used the Bad Man's Last Breath addon for the Nurse, but this issue occurs with all addons in the game.

Steps to Reproduce

1. Equip any addon that applies a status effect to any player
2. Trigger the addon's effect
3. Addon icon will be missing from the status effect notification

Explanation

When you receive a notification of a status effect that normally shows the name of an associated addon, the addon's icon is missing.
