1. Slow vault a window
2. On father Campbell's chapel

Explanation

I attempted to slow vault a window and got stuck or bugged there, other survivors couldn’t see me, pick me up or vault the same window that i did, only when the killer got pallet stunned i guess i escaped her some how but i tried to vault a window again got bugged there
