Steps to Reproduce

1. Level timer runs out, causing worms to appear
2. Got grabbed by a worm close to the elevator shaft
3. Cooper releases me, and I clip inside of the shaft and fall to the bottom
4. The fall is lethal, and Cooper is unable to reach me for a revive
5. [I suspect this would still be a big issue even if the fall wasn't lethal]

Explanation

Cooper unable to reach and revive me due to being released from a reaper worm inside of the elevator shaft before it arrived
