Cannot Pick Up Survivor with Add-Ons Equipped

Actual Result:	Hit a survivor with a hatchet throw full charge and went over to pick them up but only damage gen was popping up. so damaged the gen and still nothing, later on found the same survivor near the wall and hit them with a melee and still couldn't pick them up? (maybe hacking?)
Expected Result:	Being able to pick up said survivor and hook them

Steps to Reproduce

1. Load into Family residence
2. Hitting a survivor working on a gen with hatchet using those add-ons

Explanation

Just played as the Huntress with two of her iridescent add-ons and downed a survivor with the one shot hatchet add-on and could not pick them up to hook? (Add-ons were Iridescent head and soldier's puttee, perks were discordance, hex huntress lullaby, make your choice and mindbreakers.
