Steps to Reproduce

  1. Exile a Survivor
  2. Have the Survivor get released from exile
  3. See if the Survivor appears in the closed Nostromo room

Explanation

On Nostromo, a Survivor returned from exile can appear in the secret room when the door is closed without a way to escape until someone unlocks the door.
