All the steps to reproduce the bug are in my explanation. I have no confirmation that a specific Killer, Perks, Add-ons are required. Not enough testing on my part.

Explanation
I was playing The Mastermind now that he is back. It was a match of 1v4 and I was using the Perk: Superior Anatomy, Bamboozle, Hex: Huntress Lullaby, Thwack! and the Add-ons: Lion Medallion, Green Herb. Now for the actual bug. I am not 100% but I'm pretty sure that I had killed a survivor recently on a specific hook because I could see a flashlight on the ground. I could see scratch marks in front of the hook appearing and disappearing continuously without any survivors being around. I came back later in the match and it was still happening. The map was Springwood and the hook was next to a building behind the school near the top right corner of the map if the front of the school is near the street and the red and yellow toy car.
